I would suggest contacting the medical board and even filing a complaint, but back it up with exhibits. I've been involved with similar situations where doctors unduly billed individuals. At face value it appears this doctor wants to get even with you. I'm sure you're saving the bills, they could benefit you down the road as exhibits / evidence in moving papers.
Letter writing creates responses to you, and these responses can also be used as exhibits. When sending an important letter mail it certified including the certified number on the letter. Later you may look it up on the Internet and save the proof of delivery. I often go above the heads of the social/case worker and only converse with their supervisor, all in writing.
Request your free credit report and dispute any and all bills that are not your responsibility to pay.
If you know the area the boys are in you might fire off letters to dermatologists in that area telling them who you are and that you would like to come in to pick up copies of their records. Fish for a reply... Don't tell them about government involvement.

If you're going to be speaking with caseworker(s) on the phone make sure you digitally record. When I deal with caseworkers and/or maggots I tell them up front that I want everything done in writing to avoid any discrepancies in what is being said. Remember you're involved in litigation and hearsay evidence is allowable.
Keep attempting to get all the records you can. Request the medical records directly from the doctor if you haven't already done so. Keep requesting information... The more letters you write requesting answers the more they will reply and the more evidence you'll have.
Remember they may have taken guardianship but they've not taken your parental rights away.
